Must be at least 13 years old to complete the intake online.  If you are a parent/guardian of a child who is 12yrs old or younger, you must contact the office by phone 519-253-3100 or 1-844-900-7222 extension 2033.

To fill out the Intake Questionnaire please click on the link below and fill in every section up to and including “Therapy”.

Once your completed intake is received by SACC you will be contacted by email or phone to set up a date and time to complete a further assessment with an Intake Worker.  The worker will contact you by phone at your scheduled time to help plan for services.  SACC appreciates it is difficult to reach out for help but our resources are limited and depending on the volume of intakes this process may take a few weeks.
